Evolving Techniques of Laparoscopic Nephrectomy

Purpose: Over the past 20 years, nephron sparing surgery for solid renal lesions has been shown to have excellent technical success and oncologic outcomes similar to those of radical nephrectomy. Several larger centers have had good results applying minimally invasive techniques to nephron sparing surgery. Minimally invasive partial nephrectomy, however, remains one of the most challenging urologic procedures because of the technical difficulty of both performing an oncologically adequate resection of a tumor and achieving excellent hemostasis while at the same time minimizing renal ischemic time. Materials and Methods: This video illustrates several techniques of minimally invasive partial nephrectomy, including standard laparoscopic, hand assist, and robotic assist. It also introduces the as yet unreported technique of laparoscopic-assisted partial nephrectomy. This video is composed of clips from multiple cases performed at a single institution. Techniques shown include various methods of dissection, renal vascular control, tumor excision, and hemostasis. The video also reviews imaging modalites for optimal operative planning. Conclusion: This video will serve as a paedagogic tool to make a very technically demanding procedure more accessible.
